Report an Incident by Form Submission
There are two options available for filling out a form to report an Incident. The first is a digital form that will be sent to our team automatically upon completion. The second is a fillable PDF that you can then print off and deliver to the correct office.

Providers of services to individuals with DD are required by rule to call in and report UI/MUI within four (4) hours of the event, and submit a written report by 3 p.m. the next business day.
Because people with developmental disabilities are among our community’s most vulnerable citizens, the Preble County Board of Developmental Disabilities is committed to protecting their health and safety.
Our Investigative Agents at are responsible for examining cases of abuse, neglect and unusual incidents involving people we serve.
All reports are then reviewed by the Ohio Department of Developmental Disabilities. Incidents of a criminal nature are turned over to law enforcement.
Please provide the following information when calling to report possible abuse, neglect or other unusual incidents:
- Name of individuals involved
- Names of witnesses
- Location the incident occurred
- Details about the incident
- The action taken to remedy the situation
